Parental/Guardian Consent Form
(For participants under 16 years of age)
It is an insurance requirement that any participant under the age of 16 years must bring a completed Parental/guardian Consent Form with them on the day. Failure to provide a completed form may result in individuals being excluded from the activity.
No refunds will be given in these circumstances.
